Using Broken Windows Theory as the Backdrop for a Proactive Approach to Threat Identification in Health Care

Summary: This study introduces an alternative approach for identifying and analyzing hazardous events, focusing on threat windows which are accumulations of disruptions in order to improve the identification of system weaknesses and intervene before errors and adverse events occur, based on the broken windows theory.
