A multicentre analytical comparison study of inter-reader and inter-assay agreement of four programmed death-ligand 1 immunohistochemistry assays for scoring in triple-negative breast cancer

Summary: In this multicentre study, PD-L1-IC-positivity for SP142, 22C3 and 28-8 was found to be reproducible and analytically concordant in TNBC, suggesting analytical interchangeability. However, SP263 showed significantly higher PD-L1-IC-positivity, warranting further investigation into its relevance. Inter-reader agreement was moderate-to-strong for each assay, while inter-assay agreement varied from poor to strong.

Summary: Novel antibody-drug conjugates targeting HER2 have shown high activity in HER2-negative breast cancer with low HER2 expression, but the clinical and molecular characteristics of HER2-low breast cancer remain unclear. This study reveals that HER2-low is more common in HR-positive disease than in TNBC, and emphasizes the significant biological heterogeneity of HER2-low BC, highlighting the importance of reproducible and sensitive assays for measuring low HER2 expression.

Comparison of three FDA-approved diagnostic immunohistochemistry assays of PD-L1 in triple-negative breast carcinoma

Summary: The study compared the concordance of three PD-L1 assays in TNBC, finding that 28-8 and 22C3 had higher positive rates and good agreement in scoring methods, while SP142 showed lower concordance with the other assays. When using SP142 as a reference, optimal agreement was achieved by selecting specific cutoff values for IC, TC, and TCIC/CPS.
