Lung MRI with hyperpolarised gases: current & future clinical perspectives

Neil J. Stewart et al.

Summary: The use of pulmonary MRI has been limited in clinical settings, but advancements in ultrashort and zero echo time MRI techniques have enabled non-ionising structural imaging in certain lung disorders. Hyperpolarised gas MRI with inhaled 3He and 129Xe, a complementary technique for functional and microstructural imaging of the lung, has great potential as a clinical tool for early detection and improved understanding of pathophysiology in various lung diseases. It can also enable safe and sensitive monitoring of disease progression and response to therapy.

Computed Tomography-based Airway Surface Area-to-Volume Ratio for Phenotyping Airway Remodeling in Chronic Obstructive Pulmonary Disease

Sandeep Bodduluri et al.

Summary: The study analyzed airway remodeling in COPD patients by examining the ratio of airway luminal surface area to volume, which indicates airway narrowing and loss. The results showed that airway SA/V is associated with disease severity, lung function decline, respiratory quality, and survival in COPD patients.

Inflammatory Endotype-associated Airway Microbiome in Chronic Obstructive Pulmonary Disease Clinical Stability and Exacerbations A Multicohort Longitudinal Analysis

Zhang Wang et al.

Summary: Analyzing the airway microbiome can help stratify neutrophilic COPD into subgroups that require different treatment approaches. Patients with neutrophilic COPD are susceptible to significant microbiome shifts during exacerbations, especially those in a stable state at baseline. Monitoring the temporal variability of the airway microbiome may provide insight into patient inflammatory status over time.

Markers of disease activity in COPD: an 8-year mortality study in the ECLIPSE cohort

Bartolome Celli et al.

Summary: In COPD, changes in exacerbation frequency, systemic inflammation, BODE and SGRQ scores, and FEV1 decline over 1 and 3 years are independent markers of disease activity associated with 8-year all-cause mortality. These markers are generally independent and not predictable from baseline measurements.

Judith Garcia-Aymerich et al.

Summary: The D-PPAC and C-PPAC instruments showed strong internal consistency and construct validity in stable COPD patients, correlating with dyspnea, health-related quality of life and exercise capacity. Difficulty scores improved after pharmacological treatment and pulmonary rehabilitation, while amount scores improved after behavioral physical activity interventions.

Spirometry: A practical lifespan predictor of global health and chronic respiratory and non-respiratory diseases

Alvar Agusti et al.

Summary: The article proposes that spirometry is an overlooked global health marker that can be used to monitor human health and predict risks of chronic diseases. It emphasizes the importance of spirometry in assessing respiratory health and identifying individuals at risk of both respiratory and non-respiratory diseases. The authors suggest that spirometry can be a reproducible, non-invasive, safe, and affordable global health marker throughout the lifespan, serving as an early warning system to save lives.

Metformin: Experimental and Clinical Evidence for a Potential Role in Emphysema Treatment

Francesca Polverino et al.

Summary: This study demonstrated that metformin can protect against smoking-induced lung pathology in animal models, and in humans, metformin use is associated with a slower progression of emphysema. These findings suggest the potential of metformin in limiting emphysema progression and its systemic consequences.

The current state of tobacco cessation treatment

Humberto K. Choi et al.

Summary: Nicotine addiction is a chronic relapsing disease that requires proactive treatment with first-line medications such as nicotine replacement therapy, bupropion, and varenicline. Comprehensive smoking cessation treatment, based on individualized needs and preferences, is crucial for success. Avoiding the use of e-cigarettes and being cautious of alternative treatments are also key in tobacco cessation.

Prognostic value of clinically important deterioration in COPD: IMPACT trial analysis

MeiLan K. Han et al.

Summary: This study investigated the prognostic value of CID events in COPD patients and the impact of triple versus dual therapy on reducing CID risk. The results showed that patients with CID at week 28 had higher exacerbation rates, smaller improvements in lung function and health status at week 52, and increased risk of all-cause mortality after week 28. FF/UMEC/VI significantly reduced CID risk compared to dual therapies, potentially improving long-term prognosis in this population.
