Assessment of learning curve and oncologic feasibility of robotic pancreaticoduodenectomy: A propensity score-based comparison with open approach

Oncologic Outcomes After Robotic Pancreatic Resections Are Not Inferior to Open Surgery

Summary: This study compared oncologic outcomes for robotic and open pancreatic resections in patients with pancreatic adenocarcinoma, finding no significant differences in major complications or receipt of adjuvant chemotherapy between the two approaches. Robotic pancreatectomy resulted in shorter hospital stays, lower rates of wound infection and transfusion compared to open pancreatectomy, potentially leading to improved survival for patients. The robotic approach was also independently associated with decreased blood loss and fewer transfusions, suggesting its potential benefits in pancreatic cancer treatment.

There were no significant changes in other outcomes, and academic and non-academic centers had equivalent rates of key surgical outcomes and survival.

Cost Analysis of Pancreaticoduodenectomy at a High-Volume Robotic Hepatopancreaticobiliary Surgery Program

Learning curve of laparoscopic and robotic pancreas resections: a systematic review

Summary: This study provides a detailed summary of the current evidence on the learning curve in minimally invasive pancreatic resection, showing no significant difference between the learning curve for robotic pancreaticoduodenectomy versus laparoscopic pancreaticoduodenectomy, and robotic distal pancreatectomy versus laparoscopic distal pancreatectomy. These findings are limited by the retrospective nature and heterogeneity of the studies published to date.

Summary: Risk factors for conversion during MIPD include age, tumor size, tumor location, laparoscopic approach, and surgery in medium-volume centers. Though conversion during MIPD itself was not associated with worse outcomes, the overall outcome in these patients was generally poor, which should be taken into account when selecting patients for MIPD.
