Secretory Cells Dominate Airway CFTR Expression and Function in Human Airway Superficial Epithelia

Kenichi Okuda et al.

Summary: The study revealed that secretory cells dominate CFTR expression and function in human airway superficial epithelia, suggesting that CFTR therapies may need to restore CFTR function to multiple cell types, with a focus on secretory cells.

Modulators of CFTR. Updates on clinical development and future directions

Emmanuelle Bardin et al.

Summary: Cystic fibrosis is the most frequent life-limiting autosomal recessive disorder in the Caucasian population caused by mutations in the CFTR gene. Current therapies focus on treating the downstream consequences of CFTR mutations, but pharmacologic therapy aims to restore mutated CFTR function and has the potential to transform patient prognosis.

CRISPR/Cas9 gene editing therapies for cystic fibrosis

Carina Graham et al.

Summary: The usage of CRISPR-Cas9 in vivo for treating cystic fibrosis is hindered by the presence of non-dividing cells in the airway epithelium, which makes precise editing difficult. Future research should focus on developing more precise NHEJ-based approaches or editing strategies that do not involve introducing genomic double-strand breaks.

Changes in airway inflammation with pseudomonas eradication in early cystic fibrosis

Luke W. Garratt et al.

Summary: This study evaluated the changes in neutrophil elastase (NE) activity following Pseudomonas aeruginosa eradication in children with cystic fibrosis. The results showed that NE activity was significantly reduced post-eradication, but persisted in some subjects. High post-eradication NE levels were associated with an increased risk for P. aeruginosa infection in the next annual visit.

Entering the era of highly effective modulator therapies

Kavita Dave et al.

Summary: CFTR modulators are oral drugs that improve the function of the CFTR protein to treat cystic fibrosis, with ivacaftor as a highly effective potentiator that is available clinically worldwide. However, for common CFTR mutations like F508del, combination therapy of correctors and potentiators is required for clinical efficacy.

Immunomodulatory Effects of Azithromycin Revisited: Potential Applications to COVID-19

Vincent J. Venditto et al.

Summary: The rapid spread of COVID-19 has led to an increased focus on finding effective treatments, with many immunomodulatory therapies currently under investigation. Azithromycin, as an immunomodulatory drug, has potential benefits in late-stage treatment but also carries risks of immunosuppression and off-target effects.

Clinical Implications of Polymicrobial Synergism Effects on Antimicrobial Susceptibility

William Little et al.

Summary: Research has shown that many human infectious processes are associated with a polymicrobial burden, which can lead to treatment failure and worse patient prognoses. Current clinical standards for determining antimicrobial susceptibility only test unimicrobial suspensions, but there is growing evidence that microorganisms in a polymicrobial environment can synergize their activities and impact antimicrobial susceptibility through resistance and tolerance mechanisms.

Episodic Aspiration with Oral Commensals Induces a MyD88-dependent, Pulmonary T-Helper Cell Type 17 Response that Mitigates Susceptibility to Streptococcus pneumoniae

Benjamin G. Wu et al.

Summary: The study found that aspiration with oral commensals in mice led to short-term lower-airway dysbiosis, but induced a prolonged inflammatory response, including IL-17-producing T cells, lasting at least 14 days. Furthermore, aspiration with oral commensals decreased hosts' susceptibility to respiratory challenge with S. pneumoniae.

Summary: Single-cell RNA profiling of human cystic fibrosis proximal airway tissue shows an abundance of epithelial cells transitioning to specialized ciliated and secretory cells, coupled with a decrease in cycling basal cells. The study provides insights into disease-related changes and offers potential for the development of new targeted therapies for cystic fibrosis airway disease.
