Circadian Variation in Efficacy of Medications

Summary: Despite advancements in understanding circadian clocks and rhythms over the past few decades, translating this foundational science into clinical applications has been slow. It is suggested that double-blind clinical studies be conducted to determine the optimal time of day for medication effectiveness, with time of day being considered a key biological variable in ongoing clinical research. Additionally, it is emphasized that research design should consider time of day as a significant variable similar to sex, as historical pharmaceutical data have been skewed towards morning dosing without strong evidence of optimal efficacy.

Summary: The evidence suggests that elevated asleep blood pressure and blunted sleep-time relative blood pressure decline are significant independent prognostic markers of cardiovascular disease risk. Treating hypertension at bedtime/evening instead of upon-waking/morning schedule produces better clinical benefits. Current guidelines do not adequately address this issue.

A circadian clock regulates efflux by the blood-brain barrier in mice and human cells

Summary: The blood-brain barrier is critical for neural function, and this study shows that efflux of xenobiotics through the BBB follows a circadian rhythm in mice and human cells. Interestingly, limited circadian regulation of transcription was detected in brain endothelial cells, with no evident oscillations in efflux transporters. The molecular clock drives cycling of intracellular magnesium through transcriptional regulation, contributing to the rhythm in efflux.
