Summary: This study estimated the cost-effectiveness of different diagnostic strategies for identifying significant CAD in patients with chest pain in Australia. The most cost-effective approach was found to be using stress CMR if the exercise stress test was inconclusive or positive.

What Is the Clinical Impact of Stress CMR After the ISCHEMIA Trial?

Theo Pezel et al.

Summary: Cardiovascular mortality due to coronary artery disease has been increasing recently, despite decades of decline. The impact of non-invasive imaging strategies to guide initial coronary revascularization and improve long-term outcomes is still debated. The ISCHEMIA trial highlights the importance of optimized medical therapy and suggests potential benefits of revascularization in certain patient sub-groups.

Prognostic value of dipyridamole stress perfusion cardiovascular magnetic resonance in elderly patients >75 years with suspected coronary artery disease

Theo Pezel et al.

Summary: This study found that stress cardiovascular magnetic resonance (CMR) can effectively assess the prognostic value of cardiovascular disease in elderly individuals over 75 years old. Inducible ischemia is an independent predictor of major adverse cardiovascular events (MACE), and women without ischemia have a lower occurrence of MACE compared to men.

Summary: Cardiovascular magnetic resonance (MR) is a non-invasive imaging technique that provides a high imaging standard for studying cardiac anatomy, function, and tissue characteristics. This review comprehensive guide for clinicians on MR safety, contraindications, and image quality, covering interactions with the human body, allowed devices, image-affecting conditions, and contrast agent considerations.

Feasibility and Prognostic Value of Vasodilator Stress Perfusion CMR in Patients With Atrial Fibrillation

Theo Pezel et al.

Summary: This study aimed to assess the feasibility and prognostic value of vasodilator stress perfusion cardiovascular magnetic resonance (CMR) in patients with atrial fibrillation (AF). The presence of ischemia or late gadolinium enhancement (LGE) was found to be associated with the occurrence of major adverse cardiovascular event(s) (MACE) in AF patients, and was confirmed as independent predictors of MACE in multivariate analysis. Stress perfusion CMR is feasible and has good discriminative prognostic value to predict the occurrence of MACE in patients with AF.

Summary: ATP-induced perfusion defects are predictive of MACE, primarily driven by cardiac hospitalization, late coronary revascularization, and cardiac death. Significant independent predictors of MACE include stress-induced perfusion defect, lower LVEF, and infarct detected by LGE.

Summary: In this large multicentre registry study, it was found that elevated BMI did not negatively impact the diagnostic quality and the effectiveness of risk stratification for patients undergoing stress cardiac magnetic resonance imaging (CMR). Furthermore, BMI was shown to influence the predictive value of ischemia and late gadolinium enhancement (LGE) for adverse events.
