Right heart failure in pulmonary hypertension: Diagnosis and new perspectives on vascular and direct right ventricular treatment

Khodr Tello et al.

Summary: RV-arterial coupling is crucial for prognosis in pulmonary hypertension, but its measurement is complex and invasive. Pharmacotherapies have shown beneficial effects on RV function, but studies on RV-arterial coupling effects are limited. Simplified methods should be validated and included in clinical practice and trials.

Right Ventricular Strain Curve Morphology and Outcome in Idiopathic Pulmonary Arterial Hypertension

Roberto Badagliacca et al.

Summary: This study aimed to explore speckle tracking echocardiographic right ventricular post-systolic strain patterns in patients with idiopathic pulmonary arterial hypertension (PAH). Three distinct patterns were identified and were found to be associated with disease severity and prognosis, providing additional prognostic power when combined with clinical and hemodynamic variables.

The Growing Role of Echocardiography in Pulmonary Arterial Hypertension Risk Stratification: The Missing Piece

Cristiano Miotti et al.

Summary: Pulmonary arterial hypertension is a rare, progressive disease with poor prognosis characterized by right ventricular afterload mismatch leading to right heart failure. Treatment optimization is recommended based on regular risk assessments, and echocardiographic data may help better identify right ventricular adaptation in patients.

Prediction Models and Scores in Pulmonary Hypertension: A Review

Sophia Anastasia Mouratoglou et al.

Summary: Risk assessment in pulmonary arterial hypertension (PAH) involves various parameters derived from clinical studies and registry data. While most risk stratification scores use hemodynamic data for prognosis assessment, the discrimination ability in intermediate risk groups remains lower. The current ESC/ERS proposed risk stratification score has been validated in European patient populations but there is still a need for periodic re-evaluation and refinement to maintain clinical utility.

The Prognostic Value of Right Atrial Strain Imaging in Patients with Precapillary Pulmonary Hypertension

Summary: This study investigated the prognostic value of right atrial (RA) function by strain echocardiography in patients with pulmonary hypertension (PH), finding that RA peak longitudinal strain was significantly associated with survival and had additive prognostic value beyond traditional clinical measures like right ventricle (RV) strain, RA area, and RA pressure. This suggests that RA mechanical function assessment using strain imaging could have important clinical applications in PH patients.

Summary: The study showed that echocardiographic ARHR is an independent determinant of prognosis in IPAH at long-term follow-up, and may help indicate RV morphologic and functional improvement associated with better prognostic likelihood.
