Comparison of Two Clinical Upper Motor Neuron Burden Rating Scales in ALS Using Quantitative Brain Imaging

Summary: This study compared the relationship of two clinical UMNSs in ALS with neuroimaging markers and neurological impairment status. It was found that MGH UMNS only measures spasticity, while Penn UMNS measures spasticity, hyperreflexia, and pseudobulbar affect. Pseudobulbar affect did not map to any specific brain regional dysfunction, while hyperreflexia and spasticity correlated significantly with neurobiological changes in corticospinal pathways.
