Summary: Stroke, a global disease with high disability rate, has limited options for functional rehabilitation and impacts patients' lives adversely. Mesenchymal stem cells (MSCs) have emerged as a new focus for treatment due to their potential for neuroregeneration, showing therapeutic efficacy in cell migration, angiogenesis, immunomodulation, neuroprotection, and neural circuit reconstruction. Clinical trials on MSCs for treating ischemic stroke have demonstrated safety and feasibility, with challenges in timing and optimal dose of MSC administration. Despite ongoing research, MSC therapy holds potential as a new therapeutic approach for neurological recovery from ischemic stroke in the future.
