Lack of association between acute stroke, post-stroke dementia, race, and beta-amyloid status

Lauren N. Koenig et al.

Summary: This study aimed to determine if preclinical Alzheimer disease is a risk factor for stroke and post-stroke dementia, and whether racial differences moderate this relationship. The findings suggest that although there were some racial differences at baseline, preclinical Alzheimer disease did not show evidence of being a risk factor for stroke nor predictive of post-stroke dementia. Additionally, African Americans with clinical stroke presentations had greater levels of vascular pathology on MRI even after controlling for several vascular risk factors.

Summary: The study found that cognitive function remains impaired post-stroke, while there is modest spontaneous motor recovery over an 84-day period. Reduced neuronal loss and decreased neuroinflammation in the peri-infarct region may explain the spontaneous motor recovery, whereas persistent neuronal loss with increased neuroinflammation and amyloid-beta accumulation in the hippocampus likely contribute to the persistent cognitive dysfunction.
