Haste or Waste? Peer Pressure and Productivity in the Emergency Department

Summary: Analyzing extensive discharge data from hospital emergency departments, it was found that physicians systematically adjust their pace and intensity of care in peer group environments, leading to faster work but also fewer tests ordered and less money spent in certain groups. Contrary to the flat-of-the-curve hypothesis, these cutbacks in care actually result in significant reductions in quality of care. This evidence, together with the lack of better outcomes from slower physicians, suggests substantial differences in physician productivity within a hospital based on resource utilization.
