Summary: In sepsis-associated encephalopathy (SAE), cerebral vasculature responds first to acute systemic inflammation by upregulating immediate early response genes and activating the nuclear factor-kappa B pathway. Microglial activation occurs later than cerebral endothelial cells (CECs), suggesting that CECs are the likely initial source of proinflammatory mediators, leading to glial cell activation. This is followed by apoptotic signaling in CECs, disrupting the blood-brain barrier and causing a neuroinflammatory cascade.

Summary: This study analyzed metabolomic data from septic shock patients treated with L-carnitine and found that concentrations of branched chain amino acids (BCAA) independently predicted the time to shock resolution. BCAA concentrations were significant predictors of primary outcomes and showed potential for further investigation into the pathophysiology of cardiovascular failure in sepsis.
