National external quality assessment for next-generation sequencing-based diagnostics of primary immunodeficiencies

Kim Elsink et al.

Summary: The Dutch genome diagnostic centers (GDC) utilize NGS-based diagnostic applications for the diagnosis of primary immunodeficiencies (PIDs), facing challenges in interpreting genetic variants due to phenotypic and genetic heterogeneity. An external quality assessment among four main Dutch GDCs showed uniformity in final interpretation and conclusions of genetic analyses, emphasizing the necessity of clinical and immunological data provided by medical specialists for proper conclusions to be drawn from genetic data.

Summary: The latest classification of inborn errors of immunity/primary immunodeficiencies by the International Union of Immunological Societies Expert Committee, published in January 2020, quickly became outdated due to the rapid pace of genetic variant discoveries in the field. To bridge the gap until the next official report in 2022, researchers have highlighted 26 additional monogenic gene defects as novel causes of immune defects.

Summary: Patients with CVID and CVID-like disorders exhibit significant genetic, allelic, and phenotypic heterogeneity, making genetic testing complex. Applying the ACMG criteria to diagnose CVID and CVID-like disorders requires close collaboration between clinicians, bioinformatics, and genetics professionals.

Genetic Mosaicism as a Cause of Inborn Errors of Immunity

Jahnavi Aluri et al.

Summary: Inborn errors of immunity (IEIs) are a group of disorders caused by genetic defects in the immune response, with a diverse clinical spectrum. Diagnosis of the exact genetic cause has led to improved patient care, but remains challenging in sporadic cases. Somatic mosaicism is emerging as a significant contributor to IEI, but detecting it can be challenging with current sequencing techniques. Understanding genetic mosaicism in IEI is crucial for refining genetic analysis and disease discovery.
