Improving evidence-based grouping of transitional care strategies in hospital implementation using statistical tools and expert review

Jing Li et al.

Summary: By analyzing patient and hospital survey data and using sophisticated statistical tools, this study identified five groups of transitional care strategies commonly implemented by hospitals, which have potential to improve patient outcomes. Individual patient exposure to these strategies was found to be more predictive of readmissions than hospitals' reported delivery of strategies.

Impact of an Emergency Department-to-Home Transitional Care Intervention on Health Service Use in Medicare Beneficiaries A Mixed Methods Study

Jessica R. Schumacher et al.

Summary: This research aimed to test the hypothesis that an ED-to-home transitional care intervention reduces hospital-based acute care in chronically ill, older ED visitors. Results showed that while the intervention did not significantly reduce return ED visits or hospitalizations overall, in those with return ED visits, the Intervention Group was less likely to be hospitalized than the Usual Care Group. The study highlights the need to focus on the structural features of the health care system to reduce hospital-based acute care, rather than solely on patients' care-seeking decisions.
