Occult Hepatocellular Carcinoma Associated With Transjugular Intrahepatic Portosystemic Shunts in Liver Transplant Recipients

Summary: Transplant eligibility for hepatocellular carcinoma (HCC) is determined by imaging identification of tumor burden within the Milan criteria. Presence of pretransplant TIPS is associated with occult HCC on explant pathology, while portal vein thrombosis (PVT) similarly correlates with occult disease. However, TIPS status is not independently associated with reduced disease-free survival (DFS) or overall survival (OS) but may impact exceeding the Milan criteria in certain patients.
