Summary: Barth syndrome is a rare X-linked genetic disorder caused by mutations in the gene encoding the transacylase tafazzin, leading to loss of cardiolipin and severe cardiomyopathy. This study investigated which mitochondrial sites produce excessive oxidants in BTHS, and found that despite reduced oxidative capacity, there was no significant difference in superoxide/H2O2 production between tazkd mice and wild-type littermates. This raises questions about the role of mitochondrial oxidants in BTHS pathology.

Summary: Barth syndrome is a rare X-linked genetic disease caused by mutations in the tafazzin gene, resulting in alterations in mitochondrial phospholipid composition and disruption in mitochondrial bioenergetics. A diagnostic marker for the disease is the increased monolysocardiolipin:cardiolipin ratio in blood. Current treatment options for this disease include pharmacological management, non-pharmacological management, and promising therapeutic options such as lipid replacement therapy and gene replacement therapy.

The failing heart experiences an energy deficit due to decreased mitochondrial oxidative capacity, compensated for by increased ATP production from glycolysis. Alterations in mitochondrial ATP production fuels and fatty acid oxidation contribute to the inefficiency of the failing heart, with potential therapeutic targets in energy metabolic pathways to improve cardiac function.
