Novel porcine model of Crohn's disease anastomotic stricture suitable for evaluation and training of advanced endoscopic techniques

Martin Lukas et al.

Summary: The study created a large animal model of anastomotic stricture with Crohn's disease properties, allowing for the development of new techniques and endoscopic interventions. The model demonstrated histologically verified changes similar to CD and maintained stable diameter for over 6 months, confirming the feasibility of endoscopic therapeutic interventions.

Summary: This study identified an ileal gene program for macrophage and fibroblast activation linked to stricturing complications in treatment of naïve pediatric CD. The research may inform novel small molecule therapeutic approaches targeting macrophage and fibroblast activation and angiogenesis to reverse the stricturing gene signature.

Fibrotic Strictures in Crohn's Disease: Mechanisms and Predictive Factors

Riccardo Mager et al.

Summary: Fibrotic strictures in patients with Crohn's Disease pose a serious complication that often requires surgical resection or endoscopic dilation. However, treatment methods are associated with high recurrence rates and risks of surgical complications. There is currently limited data on biomarkers and genetic factors to predict the development of intestinal fibrosis in CD patients.

Development of a Simple, Serum Biomarker-based Model Predictive of the Need for Early Biologic Therapy in Crohn's Disease

Danny Con et al.

Summary: A simple model was developed to predict the need for early biologic therapy in Crohn's disease patients. The CICA index, based on highest CRP and lowest albumin levels, showed robust discriminative performance in predicting immunomodulator failure. External, prospective validation is needed for further evaluation.

The Medical Management of Paediatric Crohn's Disease: an ECCO-ESPGHAN Guideline Update

Summary: This study emphasizes the importance of early assessment of high-risk patients with Crohn's disease to reduce intestinal damage, and recommends upfront anti-TNF agents in combination with an immunomodulator for patients with perianal disease, stricturing or penetrating behavior, or severe growth retardation. Therapeutic drug monitoring is preferred over empirically escalating anti-TNF dose or switching therapies. For low-risk luminal CD patients, induction with exclusive enteral nutrition or corticosteroids, and maintenance therapy with immunomodulators are recommended.

Summary: DKI is effective in evaluating bowel fibrosis in patients with CD, and a new MRI-based classification using DKI and conventional MRI parameters can help distinguish between fibrotic and inflammatory intestinal strictures. The new classification shows moderate agreement with histological classification.

Application of Ultrasound Elastography for Assessing Intestinal Fibrosis in Inflammatory Bowel Disease: Fiction or Reality?

Roberto Gabbiadini et al.

Summary: Ultrasound elastography (USE) has been proposed as a non-invasive technique to assess intestinal fibrosis in patients with stricturing Crohn's disease (CD). Despite methodological heterogeneity, current studies suggest that USE can effectively evaluate the degree of fibrosis in these patients. Further research with larger sample sizes is needed before incorporating USE into standard clinical practice.

Automated Detection of Crohn's Disease Intestinal Strictures on Capsule Endoscopy Images Using Deep Neural Networks

Eyal Klang et al.

Summary: The study tested a deep learning network for detecting intestinal strictures in Crohn's disease patients' capsule endoscopy images. The network exhibited excellent accuracy in distinguishing strictures from normal mucosa as well as different degrees of ulcers, suggesting potential for automated detection and grading of Crohn's disease-related findings.

Summary: This study provides a summary of the latest advances in the treatment of stricturing Crohn's disease patients, indicating that anti-TNF drug therapy and endoscopic balloon dilation are effective strategies for avoiding surgery, while other drug therapies do not show significant benefits.
