Summary: Cystic fibrosis primarily affects the lungs, but it also has significant gastrointestinal manifestations that impact diagnosis, prognosis, and quality of life. This review discusses the various gastrointestinal issues associated with CF, including pancreatic insufficiency, gastroesophageal reflux disease, liver disease, and diseases of the large intestine. The optimization of CF therapy has led to longer life spans for CF patients, but it is important to consider the increased risk of gastrointestinal malignancies in this population. Additionally, emerging evidence suggests that the gut microbiome plays a role in CF. The introduction of CFTR protein modulators brings hope for alleviating symptoms and potentially curing gastrointestinal manifestations of CF.

Summary: Studies have shown that CF patients with pancreatic insufficiency have fecal dysbiosis. The causes of these dysbiosis are multifactorial, with important factors being antibiotic exposure, dietary intake, and CF gastrointestinal tract dysfunction. Initiating CFTR modulator treatments may lead to changes in fecal measures of malabsorption and fecal microbiomes, but further validation in larger studies is needed.

Summary: This study presents the first longitudinal, multicentre analysis of C. difficile prevalence among adult CF subjects, showing a high carriage rate of C. difficile in CF cohort. Ribotyping revealed diverse strains among different sites and varying toxin infection rates. Antibiotic susceptibility to different antibiotics also differed across the sites.
