Lung Clearance Index to Track Acute Respiratory Events in School-Age Children with Cystic Fibrosis

Lucy Perrem et al.

Summary: The study found that in school-age children with CF, LCI is more sensitive to changes during acute respiratory events compared to FEV1. A higher proportion of events showed worsening in LCI than a decline in FEV1, and combined LCI and FEV1 capture a higher percentage of events with functional impairment.

A phase 3, double-blind, parallel-group study to evaluate the efficacy and safety of tezacaftor in combination with ivacaftor in participants 6 through 11 years of age with cystic fibrosis homozygous for F508del or heterozygous for the F508del-CFTR mutation and a residual function mutation

Jane C. Davies et al.

Summary: Tezacaftor/ivacaftor demonstrated efficacy in improving lung function and CFTR function in patients aged 6 to 11 with F/F or F/RF genotypes, while also being safe and well tolerated with no new safety concerns identified.

Multiple breath washout quality control in the clinical setting

Bettina S. Frauchiger et al.

Summary: Real-time quality control (QC) of multiple breath washout (MBW) data during the measurement process is feasible in the clinical setting and leads to improved test acceptability. Before the implementation of real-time QC, only 58% of MBW tests were deemed acceptable, but this percentage increased to 75% in Bern and 70% in Zurich after the implementation of real-time QC. The agreement between MBW operators and an experienced reviewer for test acceptability was 84% in Bern and 93% in Zurich after real-time QC was implemented.

Longitudinal course of clinical lung clearance index in children with cystic fibrosis

Bettina S. Frauchiger et al.

Summary: This study described the longitudinal course of LCI in children with CF during routine clinical surveillance, finding a more pronounced increase in LCI during adolescence. Factors such as colonization with Pseudomonas aeruginosa or Aspergillus fumigatus, pulmonary exacerbations, CF-related diabetes, and bronchopulmonary aspergillosis were associated with a higher increase in LCI over time.

Improved agreement between N2 and SF6 multiple-breath washout in healthy infants and toddlers with improved EXHALYZER D sensor performance

Rikke M. Sandvik et al.

Summary: Recent studies show limited efficacy of nitrogen multiple-breath washout (N2MBW) in infants, suggesting the use of sulfur hexafluoride (SF6) MBW instead. By correcting the CO2 and O-2 interactions on gas sensors, improved N-2 signals lead to better agreement between N-2 MBW and SF6 MBW outcomes in healthy infants and toddlers. The modern EXHALYZER D system accurately calculates N-2 concentrations using O-2 and CO2 sensors.

The differing physiology of nitrogen and tracer gas multiple-breath washout techniques

Dominic Sandhu et al.

Summary: This study investigated the use of pure oxygen during nitrogen washout and its impact on lung clearance index values. The experiments showed that at low ventilation/perfusion ratios, nitrogen washout lagged behind tracer gas washout. Breathing pure oxygen introduced an asymmetry in gas flow rates, delaying the clearance of nitrogen gas.
