Finerenone and Cardiovascular Outcomes in Patients With Chronic Kidney Disease and Type 2 Diabetes

Gerasimos Filippatos et al.

Summary: The FIDELIO-DKD trial demonstrated that Finerenone reduced the incidence of cardiovascular outcomes in patients with chronic kidney disease and type 2 diabetes, with a composite outcome including cardiovascular death, myocardial infarction, stroke, and hospitalization for heart failure. There was no difference in the treatment effect based on preexisting cardiovascular disease status, with similar incidence of adverse events between treatment arms.

Epithelial Sodium Channel and Salt-Sensitive Hypertension

Summary: The development of high blood pressure is influenced by genetic and environmental factors, with high salt intake being a known environmental contributor. There is a spectrum of sodium-sensitivity among individuals, partly due to genetic variation in pathways involved in sodium handling and excretion. ENaC plays a crucial role in regulating extracellular fluid volume and blood pressure by facilitating sodium reabsorption in the distal nephron.

Renal Inflammation Induces Salt Sensitivity in Male db/db Mice through Dysregulation of ENaC

Luciana C. Veiras et al.

Summary: In male db/db mice with type 2 diabetes, salt-sensitive hypertension was observed after exposure to a high-salt diet, while female db/db mice did not develop this condition. The salt-sensitive hypertension in male mice was associated with impaired ENaC downregulation, accompanied by more immune-cell infiltration and higher levels of proinflammatory cytokines. Blocking inflammation prevented the development of salt sensitivity in male db/db mice.
