Elevated Clozapine Concentrations in Clozapine-Treated Patients with Hypersalivation

Summary: This study compared pharmacokinetic parameters between clozapine-treated patients with and without hypersalivation from a large therapeutic drug monitoring database. Patients with hypersalivation had higher clozapine plasma concentrations and concentrations-by-dose. Potential role for therapeutic drug monitoring in the prevention or management of clozapine-related hypersalivation is suggested based on the findings.

Studies show significant correlations between clozapine and norclozapine serum levels with triglycerides, heart rate, total cholesterol, and weight gain, indicating a need for further research on the cause-effect relationship between these levels and adverse drug reactions.

Summary: Clozapine is an effective antipsychotic for ultra-treatment resistant schizophrenia but limited by metabolic dysfunction. The ratio of Clozapine to its main metabolite N-desmethylclozapine is associated with insulin resistance and cognitive functioning. The study suggests that the CLZ/NDMC ratio may predict insulin resistance and metabolic comorbidity in patients with TRS receiving clozapine.

European Whites May Need Lower Minimum Therapeutic Clozapine Doses Than Those Customarily Proposed

Summary: The study compared recommended clozapine dosages in European Whites and found that the actual doses needed to reach a serum concentration of 350 ng/mL were lower than previously suggested. Further research in this population is needed to confirm and explore optimal dosing strategies for different patient subgroups.
