Summary: Three recent RCTs found that using CPAP to treat OSA did not reduce future cardiovascular events, potentially due to sample selection bias and low treatment adherence. Future studies should include sleepy individuals, maximize adherence, and consider alternative observational designs using propensity scores to overcome biases. These designs can robustly assess the cardiovascular benefit of CPAP in real-world patients and improve confidence in conclusions.
