Summary: Results from two parallel phase 2 trials comparing unrelated umbilical cord blood transplantation and HLA-haploidentical bone marrow transplantation showed similar 2-year progression-free survival rates, but lower overall survival and higher nonrelapse mortality rates after umbilical cord blood transplantation compared to haploidentical bone marrow transplantation.

Summary: Ph-like ALL is a high-risk subset of B-cell ALL in adults with poor outcomes with frontline therapy. The majority of cases harbor genetic alterations in kinases and/or cytokine receptors, leading to active research on integrating tyrosine kinase inhibitors in newly diagnosed patients and poor early responders. New therapies such as inotuzumab and blinatumomab show promising activity in chemo-refractory B-cell ALL, sparking interest in introducing these agents early in Ph-like ALL management to improve cure rates with frontline therapies.

Summary: Patients with ETP-ALL/LBL have poor survival rates, while the outcomes of near-ETP ALL/LBL are similar to non-ETP ALL/LBL. Allogeneic stem cell transplant benefits ETP-ALL/LBL but not near-ETP or non-ETP ALL/LBL. The addition of nelarabine to hyper-CVAD improves survival only in non-ETP ALL.
