Distribution of myocardial work in arterial hypertension: insights from non-invasive left ventricular pressure-strain relations

Filip Loncaric et al.

Summary: The study aimed to assess the impact of pressure on left ventricular function using MWI index, comparing MWI distribution in hypertensive patients and healthy individuals. It found that regional work impairment in hypertensive patients is unrelated to acute loading conditions, and highlighted the role of BSH as an important echocardiographic marker in hypertension.

Left ventricular myocardial work in the culprit vessel territory and impact on left ventricular remodelling in patients with ST-segment elevation myocardial infarction after primary percutaneous coronary intervention

Rodolfo P. Lustosa et al.

Summary: This study evaluated the predictive value of global and regional left ventricular myocardial work for left ventricular remodelling in patients with ST-segment elevation myocardial infarction (STEMI) before discharge. The results showed that the cardiac work index before discharge is independently associated with early adverse left ventricular remodelling in STEMI patients treated with primary percutaneous coronary intervention (PCI) and optimal medical therapy.

Summary: This study found that in patients with reduced EF, global myocardial work (GMW) can provide incremental prognostic information over EF and GLS, helping to identify patients at risk for all-cause death and HF hospitalization.

How to measure left ventricular myocardial work by pressure-strain loops

Otto A. Smiseth et al.

Summary: Myocardial work, calculated through non-invasive measures of left ventricular pressure and strain, provides diagnostic information beyond traditional indicators such as ejection fraction and strain by considering afterload and efficiency. This method can evaluate both global and segmental myocardial work, and has shown clinical value in patient selection for cardiac resynchronization therapy, with several other applications currently being tested.

Myocardial work assessment in severe aortic stenosis undergoing transcatheter aortic valve replacement

Renuka Jain et al.

Summary: By studying patients with severe aortic stenosis, it was found that the addition of mean aortic gradient to non-invasive systolic blood pressure can reliably assess myocardial work. Additionally, it was also discovered that GLS can improve post-TAVR in patients with severe aortic stenosis, while myocardial work reduces.

Summary: RVGCW is a novel parameter that provides an integrative analysis of RV systolic function and correlates more closely with invasively measured stroke volume and stroke volume index than other standard echocardiographic parameters. In patients with HFrEF, RVGWI, RVGCW, and RVGWE values were significantly lower, while values of RVGWVV were significantly higher compared to a healthy cohort.
