Whole exome sequencing in unclassified autoinflammatory diseases: more monogenic diseases in the pipeline?

Summary: In this study, phenotype-genotype correlations in unclassified autoinflammatory disease (AID) patients were investigated using whole exome sequencing (WES), revealing disease-causing genetic variants in a subset of patients. The WES strategy proved to be cost-effective and yielded substantial results for the undefined AID patients, contributing to the understanding of the spectrum of unclassified AIDs.

Clinical phenotypes and genetic analyses for diagnosis of systemic autoinflammatory diseases in adult patients with unexplained fever

Summary: In this study, 24% of Japanese patients with unexplained fever were clinically diagnosed as FMF, and genetic variants were identified in 30.1% of cases in genes other than MEFV, with these variants not considered pathogenic. Further analysis is needed to uncover the contribution of these gene variants to the inflammatory phenotypes causing fever.

Summary: This study compared the effectiveness of NGS strategy versus Sanger sequencing in genetic diagnosis of AIDs, finding a significantly higher proportion of patients with pathogenic genotypes consistent with AID phenotype in the NGS group. After the genetic report, some patients received effective treatment, and the results had an impact on the overall care of 46% of patients.

These diseases range from pure autoinflammatory conditions to primary immunodeficiencies, with a common theme of cytoskeleton defects. This emerging field provides immunologists with new insights into the physiological and pathological roles of actin cytoskeleton in immune cells.

Summary: This study found that clinical diagnostic criteria may not always be sufficient to establish the correct diagnosis, with a low accordance between clinical diagnoses and molecular analyses. In cases of patients with preliminary diagnosis of monogenic SAID and negative target gene analysis, other autoinflammatory diseases should also be considered in the differential diagnosis.
