Summary: Patients with atrial fibrillation undergoing PCI face challenges in optimal antithrombotic management due to the need for oral anticoagulation and dual antiplatelet therapy. Updated recommendations suggest the use of non-vitamin K antagonist oral anticoagulants and a double therapy approach after the peri-PCI period, with the duration of dual therapy depending on the patient's ischemic and bleeding risk profile. Triple therapy with aspirin may be considered in patients at increased thrombotic risk but with an acceptable risk of bleeding.
