Surrogate Satisfaction with Decision Making After Intracerebral Hemorrhage

Savina Sahgal et al.

Summary: The study on surrogate decision makers of patients with intracerebral hemorrhage showed that most surrogates were satisfied with the decision-making process and experienced low levels of regret. Physician treatment recommendations, shared decision making, younger patient age, higher ICH score, and spouse relationship were all associated with higher satisfaction levels. Factors such as timing of DNR orders did not significantly impact satisfaction levels.

Prevalence, Predictors, and Outcomes of Prolonged Mechanical Ventilation After Endovascular Stroke Therapy

Hamidreza Saber et al.

Summary: The study found that approximately 14.9% of patients undergoing endovascular treatment (EVT) for acute ischemic stroke required prolonged mechanical ventilation, with heart failure and diabetes being independent predictors. Prolonged ventilation was associated with a significant increase in in-hospital mortality and morbidity.

Clinicians' Perceptions of the Appropriateness of Neurocritical Care for Patients with Spontaneous Intracerebral Hemorrhage (ICH): A Qualitative Study

Siobhan Mc Lernon et al.

Summary: This qualitative study conducted semi-structured interviews with neurocritical care doctors and nurses to explore perceptions of neurocritical care in relation to expected functional outcomes for ICH patients. The study identified key themes including prognostic uncertainty, subjectivity of good versus poor outcomes, perceived inappropriate care situations, challenging nature of decision-making, and clinician distress. Awareness of these challenges could facilitate interventions to improve decision-making and reduce distress for clinicians caring for severely affected ICH patients, particularly frail elderly patients.
