Summary: This study aimed to elucidate the symptom-ameliorating effect of vonoprazan (VPZ) by evaluating the change in the pH value of the refluxate using multichannel intraluminal impedance and pH (MII-pH) monitoring. Results showed that VPZ can improve symptoms in patients with PPI-refractory nonerosive reflux disease (NERD) by increasing the pH value of the refluxate.

Summary: Almost half of patients diagnosed with reflux hypersensitivity (RH) have behavioral disorders, including excessive supragastric belching (SGB) or rumination. These behavioral disorders are associated with typical reflux symptoms in RH patients. Screening for behavioral disorders in RH patients may have important therapeutic implications.

Summary: The reduction of reflux episodes to physiological levels on pH-impedance monitoring is associated with improved outcomes in patients with regurgitation-predominant GERD. Treatment with magnetic sphincter augmentation significantly improved symptom and satisfaction scores, while also reducing acid exposure time and reflux episodes. Improvement in regurgitation score remained independently predictive of satisfaction with therapy, and a reduction to <40 reflux episodes was significantly associated with symptom response.

Ambulatory Reflux Monitoring Guides Proton Pump Inhibitor Discontinuation in Patients With Gastroesophageal Reflux Symptoms: A Clinical Trial

Summary: This study showed that in patients with typical reflux symptoms, inadequate PPI response, and absence of severe esophagitis, acid exposure on reflux monitoring could predict the ability to discontinue PPIs without symptom escalation. Symptom burden reduction was greater among those who discontinued PPI compared to those who resumed. It is suggested that upfront reflux monitoring off acid suppression can help limit unnecessary PPI use and guide personalized management.
