Haglund syndrome, consisting of Haglund's deformity, retrocalcaneal bursitis, and achilles tendinopathy, is a common cause of hind foot pain in adults. Diagnosis using planar scintigraphy can be challenging, but SPECT/CT can play a key role in its diagnosis.

We present a case of Haglund syndrome and show the key role that SPECT/CT can play in its diagnosis.
