Removing endobronchial needle-like foreign bodies in two school-age children

Xiaoxu Zhang, Qiulan Shi

Summary: Two school-age children were diagnosed with endobronchial foreign bodies (EFBs) caused by accidentally inhaling metal-containing foreign bodies. In case 1, a needle-like foreign body was found at the entrance of the right upper lobe bronchus, while in case 2, it was located in the posterior basal segment of the right lower lobe. The EFB in case 1 was successfully removed using rigid bronchoscopy. In case 2, the EFB was initially inaccessible via fiberoptic bronchoscopy, but was eventually removed by rigid bronchoscopy after accidental displacement during thoracotomy. The surgical approach should be tailored to the specific characteristics of the foreign body to minimize patient injury.

Hiroya Yonaga, Kosuke Uno, Shinichi Taniai, Masahiko Seki, Naoki Matsuno, Akihiro Shiotani, Koji Araki

Summary: Carbon ion radiotherapy (CIRT) is effective in treating adenoid cystic carcinoma, but salvage transoral surgery (TOS) after CIRT requires careful postoperative observation due to potential complications. We report a case of massive pharyngeal bleeding after salvage TOS in a patient previously treated with CIRT, which was successfully managed with surgical debridement and cauterization. It highlights the importance of evaluating the efficacy and safety of salvage TOS in patients who have undergone CIRT in the future.

Simultaneous repair of bilateral temporal bone meningoencephaloceles by combined mastoid-middle cranial fossa approach

Kazuto Osaka, Takayuki Okano, Masahiro Tanji, Koichi Omori

Summary: Meningoencephalocele (MEC) is a rare clinical condition where the dura mater and brain tissue herniate through skull base defects. In this report, we present a case of bilateral MECs discovered after recurrent bacterial meningitis. The patient underwent simultaneous bilateral resection and repair using a combined mastoid-middle cranial fossa approach, with reconstruction of bone and dura using free bone grafts and a temporalis fascia flap. Three years after surgery, the reconstructed tegmen was well preserved and there was no recurrence of meningitis or MECs. The combined mastoid-middle cranial fossa approach appears to be a promising option for treating bilateral multiple MECs.
