Post-transplantation Lymphoproliferative Disorder (PTLD): In the Liver Transplant Recipient

Asisha M. Janeela, Nambiathayil A. Fouzia, Uday G. Zachariah

Summary: Post-transplantation lymphoproliferative disorders (PTLD) are rare neoplasms that occur in the post-transplantation period. Children undergoing liver transplantation are at higher risk of PTLD. Risk factors include immunosuppression and Epstein-Barr virus status. Diagnosis requires clinical suspicion, radiological evaluation, and tissue biopsy. Risk reduction involves minimizing aggressive immunosuppressive regimens and early treatment.

Primary Sclerosing Cholangitis Epidemiology, Diagnosis, and Presentation

Aalam Sohal, Sanya Kayani, Kris Kowdley

Summary: Primary sclerosing cholangitis (PSC) is an immune-mediated liver disease characterized by inflammation and fibrosis of bile ducts. It is commonly associated with inflammatory bowel disease, especially ulcerative colitis. The natural history of the disease varies among patients, but most will develop cirrhosis.

Treatment of Autoimmune Hepatitis

Aparna Goel, Paul Kwo

Summary: The goal of autoimmune hepatitis treatment is achieving clinical and biochemical remission. Standard care involves induction treatment with corticosteroids and gradual tapering of steroids. Alternatives to standard therapy can be considered for patients with intolerance or inadequate response. Treatment withdrawal is achievable in less than 20% of patients after 2 years of sustained remission, and liver transplantation should be considered for patients with progressive disease or complications.

Alcohol-associated liver disease: Epidemiology and management

Helena Hernandez-Evole, Natalia Jimenez-Esquivel, Elisa Pose, Ram on Bataller

Summary: Alcohol is the leading cause of preventable morbidity and mortality worldwide, and the most frequent cause of liver disease. Alcohol-associated liver disease has a complex pathogenesis, with unknown precise mechanisms. Currently, there are no targeted therapies to interfere with the disease's progression, making alcohol abstinence the most effective measure to improve prognosis in patients with this disease.

MZF1 promotes tumour progression and resistance to anti-PD- L1 antibody treatment in hepatocellular carcinoma

Anna Kan, Shuang Liu, Minke He, Dongsheng Wen, Haijing Deng, Lichang Huang, Zhicheng Lai, Yexing Huang, Ming Shi

Summary: This study reveals the correlation between the transcription factor MZF1 and the immune checkpoint molecule PD-L1 in hepatocellular carcinoma (HCC). The overexpression of MZF1 is associated with an immunosuppressive environment and decreased T-cell infiltration in HCC. Mechanistically, MZF1 accelerates PD-L1 ubiquitination via the CDK4 pathway. Inhibition of CDK4 may restore PD-L1 expression and enhance the efficacy of anti-PD-L1 antibodies.

Factors Affecting and Promoting Health-related Quality of Life in Patients With Liver Cirrhosis: An Underestimated Domain in Patient Care

Siddheesh Rajpurohit, Balaji Musunuri, Pooja B. Mohan, Ganesh Bhat, Shiran Shetty

Summary: This study identified various factors affecting and promoting HRQoL in patients with liver cirrhosis, including age, gender, income, BMI, psychological factors, and complications. Physical exercise, liver transplant, stem cell therapy, and some medications were associated with improved HRQoL. More intervention trials and consideration of HRQoL in medical therapy are recommended.

Role of Computed Tomography Perfusion in Patients with Liver Cirrhosis and Hepatocellular Carcinoma

Kartik P. Ganga, Pankaj Gupta, Naveen Kalra, Arunanshu Behra, Rakesh Kapoor, Ajay Duseja, Yogesh Chawla, Manavjit S. Sandhu

Summary: This study aimed to evaluate the association between CT perfusion parameters in cirrhotic patients with and without HCC. The results showed that even the remote background liver parenchyma in patients with HCC had increased hepatic arterial perfusion. The perfusion parameters were also found to be significantly altered with increasing severity of cirrhosis.

Liver Severity Score-Based Modeling to Predict Six-Week Mortality Risk Among Hospitalized Cirrhosis Patients With Upper Gastrointestinal Bleeding

Rochelle Wong, Adam Buckholz, Kaveh Hajifathalian, Catherine Ng, Evan Sholle, Gaurav Ghosh, Russell Rosenblatt, Brett E. Fortune

Summary: This study aims to evaluate and recalibrate the accuracy of Child-Turcotte-Pugh (CTP) and Model-for-End-Stage-Liver Disease (MELD) scores in predicting six-week mortality risk. The results showed that both CTP and MELD-based models have excellent discrimination in predicting six-week mortality for all types of bleeding sources, but only the CTP-based model demonstrates calibration for all bleeding etiology.
