Hepatoid adenocarcinoma is a rare variant of adenocarcinoma that can occur in the stomach. Because of the low incidence of the disease, F-18 fluorodeoxyglucose positron emission tomography with computed tomography (FDG PET/CT) imaging studies for hepatoid adenocarcinoma of the stomach are scarce. We report the case of a 79-year-old man presenting with hepatoid adenocarcinoma of the stomach associated with hepatic metastases mimicking hepatocellular carcinoma on FDG PET/CT images. The possible presence of metastatic hepatoid adenocarcinoma should be considered in older patients with elevated serum alpha-fetoprotein and hepatic tumors with underlying chronic liver disease.
