Normal talar anatomy is mandatory for efficient and pain-free ambulation. When the talus is injured in a talar neck fracture, devastating complications can occur. Talar neck injuries present a challenge to even the most experienced foot and ankle surgeons. The authors review talar neck fractures, including history, mechanism of injury, treatment options, and potential complications. The importance of anatomic reduction with rigid internal fixation and avoidance of varus malunion is emphasized.
