Fractures of the proximal humerus are rare injuries in childhood accounting for less than 2% of all pediatric fractures. Because of the enormous remodeling potential of the proximal humeral physis coupled with the extensive mobility of the glenohumeral joint, proximal humerus fractures rarely require operative intervention in children under 12 despite significant malalignment. The goal of treatment is immobilization of the fracture and pain control for the child during healing. The limited indications for operative fixation of these fractures are discussed.
