Summary: Bariatric surgery can improve small nerve fibers and glomerular hyperfiltration in obese patients with T2D, which are associated with weight loss, triglycerides, and systolic blood pressure, but there is no change in retinopathy or urinary albumin/creatinine ratio after 12 months of surgery.

A systematic review and meta-analysis of the serum lipid profile in prediction of diabetic neuropathy

Summary: The meta-analysis found that diabetic neuropathy (DN) patients had higher triglyceride (TG) levels and lower high-density lipoprotein cholesterol (HDL) levels compared to controls, with no significant differences in total cholesterol (TC) and low-density lipoprotein cholesterol (LDL) levels. Subgroup analysis showed that the serum TG levels were elevated in type 1 diabetes mellitus (T1DM) neuropathy patients, while only T1DM neuropathy patients had reduced serum HDL levels. TC and LDL levels did not significantly differ between DN patients and controls. Additionally, higher TG and LDL levels were associated with an increased risk of DN, while higher HDL levels were associated with a reduced risk of DN. TC levels did not significantly affect the risk of DN.

Bariatric surgery leads to an improvement in small nerve fibre damage in subjects with obesity

Summary: The study showed that obese individuals often have risk factors for neuropathy, and bariatric surgery can improve these risk factors and help alleviate neuropathy. After bariatric surgery, patients experienced improvements in neuropathic symptoms and deficits, including regeneration of small nerve fibers, without significant changes in quantitative sensory testing, autonomic function, or neurophysiology.

Summary: Hyperlipidaemia is associated with neuropathy development and progression, with lipid-modifying agents potentially being a therapeutic option. Dyslipidaemia has been shown to have a detrimental effect on nerve fibres, especially hypertriglyceridemia. Clinical trials targeting hyperlipidaemia have reported varying effects with statins and fibrates. Further research is needed to establish the impact of lipid-lowering therapies on nerve fibre regeneration and neuropathy symptoms.

Diagnosis of Neuropathy and Risk Factors for Corneal Nerve Loss in Type 1 and Type 2 Diabetes: A Corneal Confocal Microscopy Study

Summary: Corneal confocal microscopy (CCM) has shown to be more effective in identifying severe corneal nerve loss in patients with type 1 diabetes compared to type 2 diabetes. It also demonstrates good diagnostic accuracy for diabetic peripheral neuropathy (DPN). Furthermore, the risk factors for a reduction in corneal nerve fiber length differ between type 1 and type 2 diabetes.
