Orthopaedic surgeons are likely to encounter patients in an austere environment during wartime or disaster. Medical teams in charge of pediatric care in these conditions must have an appropriate safety plan in place as well as medical training, medical supplies, and an understanding of the culture and environment in which they will serve. Ethical dilemmas must be anticipated, as providers may encounter times when the type of surgical care they would render in their daily practice is not possible. Care in a resource-constrained environment requires innovation and skill in a way that most surgeons have not had to function. Flexibility and adaptability are necessary to react to the unanticipated challenges that will inevitably occur. Challenges should be approached with preparation, innovation, honesty, and humility, always keeping the best interest of the patient in mind. Treatment in an austere environment is the most demanding yet rewarding experience one may encounter as a medical provider.
