OBJECTIVES Fixed rehabilitation of edentulous atrophic maxillary jaws, often represents a challenge for clinicians. Despite, recently, regenerative and bone augmentation techniques have widespread, subperiosteal implant techniques cannot be considered outdated. Indeed, the advent of cone beam digital radiology, the introduction of stereolithographic models, the use of titanium implant framework with advanced addition techniques and, consequently, the increase in precision of the manufact, give value and predictability to subperiostal implant technique. Advantages such as one-stage-surgery and reduced costs for the patient are worth mentioning. MATERIALS AND METHODS The present case report proposes a complete upper rehabilitation of atrophic jaw through the use of subperiosteal implant technique. RESULTS Three-year follow up shows maintenance of the result. CONCLUSIONS At present, the choice of the patient, the design and construction of the device, rigid fixation and the presence of adequate soft tissues around implants seem fundamental criteria for success of treatment.
