Summary: In rural communities, the COVID-19 pandemic has varying impacts on the mental health, substance use, and overdose concerns of people who use drugs. Participants reported increased feelings of boredom, loneliness, depression, worry, stress, and suicidal ideation due to COVID-19. While some reported decreased substance use, others reported increased use, with differing impacts depending on the type of substance used. Despite increased risky use and suicidal ideations, most participants were not concerned about overdose, and COVID-19 did not impact their concerns about overdose.
