Summary: This study found that patients with EOS AECOPD had better clinical outcomes compared to non-EOS AECOPD patients, with significant advantages in terms of length of hospital stay, in-hospital mortality, FEV1, and risk of arrhythmias.

Association Between Blood Eosinophils and Mortality in Critically III Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease: A Retrospective Cohort Study

Jia Yang et al.

Summary: Increased blood eosinophils were associated with decreased in-hospital mortality and shorten hospital length of stay in critically ill patients with acute exacerbation of chronic obstructive pulmonary disease. A discriminatory eosinophil threshold of 0.35% for mortality was found, but further studies were needed to verify it.

Summary: This study evaluated the predictive value of blood eosinophils in acute exacerbation of COPD, finding a correlation between elevated eosinophils and better short-term outcomes in patients with a history of smoking. However, eosinophil levels alone cannot confidently predict prognosis.

Variability of blood eosinophil count and prognosis of COPD exacerbations

Summary: The study found no significant relationship between blood eosinophil count (BEC) and hospital stay, readmissions, deaths during admission, need for intensive care, or frequent exacerbator status in COPD patients following hospital admission.

Blood eosinophils on hospital admission for COPD exacerbation do not predict the recurrence of moderate and severe relapses

Balazs Csoma et al.

Summary: The study found no difference in time to next exacerbation between eosinophilic and non-eosinophilic groups in patients with AE-COPD. However, higher number of past severe exacerbations, lower FEV1 at discharge, and higher pack-years were related to shorter time to exacerbation. Subgroup analysis showed that a significant proportion of patients with initial eosinophilic exacerbations experienced non-eosinophilic relapses on readmission. Eosinophilic severe exacerbations present a variable phenotype.

Summary: The heterogeneity of COPD brings challenges in diagnosis and management, while using blood eosinophil counts as a biomarker for ICS treatment has potential benefits but remains controversial in the respiratory community.
