Summary: An organ-preserving strategy with selective total mesorectal excision could be a feasible treatment option for early-stage rectal cancer, achieving high levels of organ preservation with relatively low morbidity and indications of improved quality of life. Further evaluation is needed to assess the long-term oncological outcomes of organ preservation in comparison to radical surgery.
