Early-Onset Osteoporosis

Outi Makitie et al.

Summary: Osteoporosis is a skeletal disorder affecting mainly the elderly, characterized by low bone density and fragility fractures. In children and young adults, investigations should explore the possibility of underlying monogenetic bone diseases when fragility fractures occur. Management involves treating secondary factors, optimizing lifestyle factors, and discussing personalized bone-active medication based on severity of osteoporosis and underlying diseases.

Mechanisms of Bone Fragility: From Osteogenesis Imperfecta to Secondary Osteoporosis

Ahmed El-Gazzar et al.

Summary: The strength of bone material is determined by various factors, and understanding the mechanisms of rare genetic bone fragility disorders not only advances medical knowledge but also may lead to drug development for more common disorders. The main disease mechanisms underlying bone fragility associated with low bone mass include type I collagen processing, WNT signaling, TGF-β signaling, the RANKL-RANK system, and the osteocyte mechanosensing pathway.

Studying bone mineral density in young people: The complexity of choosing a pQCT reference database

A. D. Lalayiannis et al.

Summary: The study found that peripheral quantitative computed tomography (pQCT) is a useful tool for studying trabecular and cortical compartments separately. However, there are variations in pQCT scanning protocols, analysis methodology, and a paucity of reference data.

Osteoclasts recycle via osteomorphs during RANKL-stimulated bone resorption

Michelle M. McDonald et al.

Summary: Research has shown that RANKL-stimulated osteoclasts can undergo fission to form osteomorphs, which are involved in the regulation of bone resorption and could be targeted for the treatment of skeletal diseases. Single-cell RNA sequencing revealed that osteomorphs are transcriptionally distinct from osteoclasts and macrophages, expressing non-canonical osteoclast genes associated with bone phenotypes when deleted in mice.

Collagen transport and related pathways in Osteogenesis Imperfecta

Lauria Claeys et al.

Summary: Osteogenesis Imperfecta (OI) is a heterogeneous disease with varying degrees of severity characterized by bone fragility, deformities, and phenotypic variation. In addition to collagen type I genes, new genetic causes have been discovered, highlighting the complexity of collagen biosynthesis and the emerging role of chaperone transport in OI development. This review emphasizes the significance of transport disruptions in the OI mechanism and aims to identify potential therapeutic targets through exploration of skeletal fragility pathways.

Part I: Which Child with a Chronic Disease Needs Bone Health Monitoring?

Leanne M. Ward

Summary: This review emphasizes the importance of diagnosing and treating underlying conditions affecting pediatric bone health, especially in identifying risk factors and fragility fractures. With an increased focus and understanding of the characteristics of fragility fractures in at-risk children, there has been a shift in treatment approach from a bone mineral density-centric to a fracture-focused method.

A Roadmap to Gene Discoveries and Novel Therapies in Monogenic Low and High Bone Mass Disorders

Melissa M. Formosa et al.

Summary: Genetic disorders of the skeleton, particularly rare bone mass disorders, have advanced our understanding of molecular signaling pathways in bone development, inheritance patterns, and the identification of new drug targets. Functional validation of novel disease genes in multiple model systems is crucial for future treatments, facilitated by extensive phenotyping and genotyping approaches.

The first experience of denosumab therapy on patients with Ehlers-Danlos syndrome and osteoporosis: detailed observation of two patients

Sako Yasukawa et al.

Summary: This study investigated the efficacy and safety of denosumab treatment in two patients with EDS and osteoporosis. Both patients showed gains in bone mineral density after denosumab therapy without experiencing fractures or other complications, suggesting denosumab could be a potential treatment option for EDS patients with osteoporosis.

Osteocytes but not osteoblasts directly build mineralized bone structures

Ke Wang et al.

Summary: This study found that osteocytes, not osteoblasts, directly build mineralized bone structures, and that osteoblasts in Dmp1-null or Hyp mice produce aberrant and poorly mineralized bone. Additionally, the constitutive expression of beta-catenin in osteocytes can recapitulate a similar osteomalacia phenotype as seen in Dmp1 null or Hyp mice. Thus, the conclusion is that osteocytes directly build bone, challenging the existing dogma.
