Current Practice of Fluid Maintenance and Replacement Therapy in Mechanically Ventilated Critically Ill Children: A European Survey

Ismail Arrahmani et al.

Summary: Appropriate fluid management in mechanically ventilated critically ill children is a challenge in pediatric intensive care medicine. Studies show a link between fluid overload and adverse outcomes, but there is no international consensus on fluid management strategies. A survey among PICU specialists in Europe found that most respondents believe fluid overload is common and associated with adverse outcomes in mechanically ventilated children. However, there is great heterogeneity in current clinical practice to address this issue.

Risk of Electrolyte Disorders in Acutely Ill Children Receiving Commercially Available Plasmalike Isotonic Fluids A Randomized Clinical Trial

Saara Lehtiranta et al.

Summary: In a randomized clinical trial conducted at the pediatric emergency department of Oulu University Hospital in Finland, commercially available plasmalike isotonic fluid therapy significantly increased the risk of clinically significant electrolyte disorders, especially hypokalemia, in acutely ill children compared to previously widely used moderately hypotonic fluid therapy containing 20 mmol/L of potassium.

Fluid-induced harm in the hospital: look beyond volume and start considering sodium. From physiology towards recommendations for daily practice in hospitalized adults

Niels Van Regenmortel et al.

Summary: Iatrogenic fluid overload, primarily caused by sodium administration, can have detrimental effects on hospitalized patients. Strategies such as using hypotonic maintenance fluids and minimizing sodium caused by fluid creep can help mitigate the risks associated with excessive sodium intake.

Association between the use of balanced fluids and outcomes in critically ill children: a before and after study

Matthew F. Barhight et al.

Summary: After implementing balanced fluids as standard care in the pediatric intensive care unit, there were no differences in rates of day 3 acute kidney injury or other clinical outcomes. However, lower rates of hyperkalemia and hyperchloremia were observed, while higher rates of hypokalemia and hypochloremia were found. Further research is needed to evaluate the impact of balanced fluids and electrolyte abnormalities in critically ill children.
