Thyroid Disorders Following Hematopoietic Stem Cell Transplantation in Childhood: Impact of Conditioning Regimen on Thyroid Dysfunction, Volume Changes, and Occurrence of Nodules

To Lighten the Burden of Cure: Thyroid Disease in Long-Term Survivors After TBI Conditioning for Paediatric ALL

Summary: Thyroid disorders are a serious complication after hematopoietic stem cell transplantation, with a prevalence of 15-30% in pediatric survivors. Factors such as total body irradiation and immunological dysregulation are associated with the development of these disorders. However, chronic graft-versus-host disease, prolonged immunosuppression, and additional radiotherapy did not show any correlation with thyroid disease. In patients with structural changes, there was a significant correlation between immunological dysregulation and the development of thyroid dysfunction.

Incidence of thyroid dysfunction in children after HSCT with reduced intensity conditioning (RIC) or myeloablative conditioning (MAC)

Summary: After implementing a late effects screening program, it was found that 10% of pediatric hematopoietic stem cell transplant recipients developed thyroid dysfunction post-transplant, with a lower incidence in those who received a RIC regimen compared to a MAC regimen. Among the subjects who developed thyroid disease, most were asymptomatic. Further follow-up will help identify risk factors for post-transplant-associated thyroid dysfunction.
