Summary: Acute respiratory distress syndrome (ARDS) is a common clinical syndrome of acute respiratory failure caused by diffuse lung inflammation and edema. The pathophysiology of ARDS is complex and involves multiple overlapping and interacting pathways of injury, inflammation, and coagulation. Mechanical ventilation can contribute to lung injury and inflammation. Resolution of inflammation requires coordinated downregulation of proinflammatory pathways and upregulation of anti-inflammatory pathways.

Longitudinal respiratory subphenotypes in patients with COVID-19-related acute respiratory distress syndrome: results from three observational cohorts

While no consistent subphenotypes were identified at baseline, two distinct subphenotypes were found during the first 4 days of invasive mechanical ventilation. These subphenotypes demonstrated differences in patient outcomes, indicating the importance of subtyping in the progression and outcomes of ARDS patients.
