Timing of Kidney Support Therapy in Acute Kidney Injury: What Are We Waiting For?

Josee Bouchard et al.

Summary: The optimal timing of kidney support therapy in critically ill patients with acute kidney injury (AKI) without life-threatening complications related to AKI is controversial. This article reviews the concept of timing of dialysis in AKI, performs a critical assessment of the most important clinical trials in this topic, discusses ongoing research and knowledge gaps, and defines key research issues to address in the future.

Patients with Severe Lactic Acidosis in the Intensive Care Unit: A Retrospective Study of Contributing Factors and Impact of Renal Replacement Therapy

Lyssa Van de Ginste et al.

Summary: Hyperlactatemia is a common condition in the intensive care unit, with control of the triggering condition being the most effective treatment. RRT is often considered as a last resort. Analysis showed a 34.5% 90-day mortality rate in patients with severe lactic acidosis, with those started on RRT having a higher odds of mortality.

On estimation and cross-validation of dynamic treatment regimes with competing risks

Pawel Morzywolek et al.

Summary: The optimal timing for initiating renal replacement therapy in patients with acute kidney injury remains challenging. This study explores different timing strategies based on serum potassium, pH, and fluid balance, using routinely collected observational data. Statistical techniques are employed to evaluate the impact of dynamic treatment regimes, considering ICU discharge as a competing event. Two approaches - nonparametric and semiparametric - are discussed, along with a cross-validation technique to assess out-of-sample performance.

Timing of dialysis initiation to reduce mortality and cardiovascular events in advanced chronic kidney disease: nationwide cohort study

Edouard L. Fu et al.

Summary: The study identified that initiating dialysis at an eGFR of 15-16 mL/min/1.73 m² was associated with the lowest five-year mortality risk and risk of major adverse cardiovascular events in patients with advanced chronic kidney disease.

The importance of the urinary output criterion for the detection and prognostic meaning of AKI

Jill Vanmassenhove et al.

Summary: This study demonstrates that ignoring the urinary output criterion in diagnosing AKI can result in missed and delayed diagnoses, as well as underestimation of the association between AKI and ICU mortality.
