Thrombectomy in special populations: report of the Society of NeuroInterventional Surgery Standards and Guidelines Committee

Fawaz Al-Mufti et al.

Summary: Mechanical thrombectomy can be effective for treating ELVO in ischemic stroke in special populations such as those over 80, under 18, with thrombocytopenia, pregnant, or with endocarditis. While outcomes may not be as favorable as in younger or normal platelet count patients, there is still potential benefit, especially in the elderly. Limited data is available for patients with collagen vascular diseases, but in carefully selected cases, MT may still be appropriate.

Summary: The meta-analysis showed that the use of DOACs in cancer patients with AF was associated with reduced risk of systemic embolism or any stroke, as well as major bleeding, compared to VKAs. However, the reduction in risk for major bleeding or non-major clinically relevant bleeding was not statistically significant when compared to VKAs. Further studies are needed to validate these findings.

Summary: Baseline thrombocytopenia (TP) in patients with acute ST-segment elevated myocardial infarction (STEMI) is associated with higher rates of 2-year all-cause death and major adverse cardiovascular and cerebrovascular events (MACCE) compared to patients with a normal platelet count (PLT). However, after multivariate adjustment and controlling for baseline differences, baseline TP is not independently associated with 2-year adverse outcomes in patients with STEMI.

Cancer Therapy-Associated Thrombosis

Steven P. Grover et al.

Summary: Cancer patients are at a higher risk of arterial and venous thrombotic events compared to the general population, with factors such as cancer site and stage, treatment-related factors, and cytotoxic effects of chemotherapeutic agents contributing to this increased risk. Despite advancements in targeted therapies, some anticancer agents still carry a risk of thrombosis, necessitating a better understanding of the mechanisms involved to develop effective treatment strategies.

Results showed varying incidence of thrombocytopenia across different tumor and chemotherapy types, highlighting the importance of predicting and preventing thrombocytopenia to avoid dose modifications and adverse effects on survival.

Nir Livneh et al.

Summary: Managing anticoagulation in hematological malignancy patients with atrial fibrillation and thrombocytopenia is a clinical challenge with limited data. Retrospective cohort study found that stopping anticoagulation may be associated with younger age and recent diagnosis of atrial fibrillation. The 30-day bleeding risk may outweigh the risk of arterial thromboembolism in this patient population.

Summary: Cancer patients with thrombocytopenia require platelet transfusions for management, but there are still unmet needs such as high residual bleeding rates and dose reductions in anti-cancer treatment.
