Disease-related malnutrition in chronic kidney disease

Summary: This review discusses the development, screening, and treatment of disease-related malnutrition in patients with chronic kidney disease (CKD). Recent research has identified changes in taste and smell and effects of polypharmacy as potential causes for malnutrition in CKD. Studies have also explored different screening and assessment tools in relation to the new Global Leadership Initiative on Malnutrition (GLIM) criteria, as well as the use of low protein diets and pre and probiotics. Furthermore, the importance of nutritional support and exercise during dialysis is being examined. Further research is needed to better understand the complexities of disease-related malnutrition in CKD and develop interventions that reflect its multifaceted pathophysiology and impact.

Summary: The review highlighted the lack of standardized treatments for cachexia or severe wasting and the need for multimodal interventions in addressing these conditions. While the studies showed some improvements in body composition, nutrition, biomarkers, and functionality in treatment groups, caution is required due to the heterogenous nature of interventions and small sample sizes. Overall, there is support for the role of multimodal interventions in treating severe wasting, but further randomized controlled trials with a powered sample size and sufficient interaction period are necessary to determine their effectiveness.

Using a generic definition of cachexia in patients with kidney disease receiving haemodialysis: a longitudinal (pilot) study

Summary: Cachexia is common in patients with chronic kidney disease receiving haemodialysis, but a uniformed disease-specific definition is still lacking. This longitudinal study followed patients for 12 months and found that a subset of patients exhibited symptoms of cachexia, including weight loss and clinical characteristics. Results showed deterioration in weight, muscle mass, and other indicators in cachectic patients.
