Tisagenlecleucel outcomes in relapsed/refractory extramedullary ALL: a Pediatric Real World CAR Consortium Report

Vanessa A. Fabrizio et al.

Summary: CAR T cell therapy has significantly improved therapeutic options for relapsed/refractory B-cell acute lymphoblastic leukemia, but data in extramedullary involvement are limited. Clinical outcomes for isolated CNS disease are encouraging, with comparable toxicity, relapse, and survival rates to patients with BM-only disease. Treatment with tisagenlecleucel is effective for relapsed/refractory EM disease, and active CNS disease at the time of infusion does not impact outcomes.

Long-term neurocognitive and quality of life outcomes in survivors of pediatric hematopoietic cell transplant

Natalie L. Wu et al.

Summary: Although long-term survivors of pediatric HCT reported higher rates of impairment in specific neurocognitive domains, cognitive quality of life was perceived as similar to the general population. Subsets of survivors with certain co-morbidities had substantially worse neurocognitive outcomes.

A review of psychological symptoms and post-traumatic growth among adolescent and young adult survivors of childhood cancer

Amy M. Berkman et al.

Summary: Childhood cancer survivors face a risk of psychological late effects, including anxiety, depression, and psychological distress, with a higher occurrence rate compared to the general population. Further investigation is needed regarding treatment, environmental, and behavioral risk factors, as well as the onset and trajectory of these symptoms.

Glucocorticoids and the Brain after Critical Illness

Alice R. Hill et al.

Summary: Treatment for critical illness often focuses on short-term physical recovery, but recent research suggests long-term risks of cognitive impairment and psychiatric disorders in survivors, potentially linked to glucocorticoid levels. Elevated glucocorticoids can cause structural changes in the hippocampus and prefrontal cortex, impacting memory and cognition in critical illness survivors.

Late outcomes in survivors of childhood acute myeloid leukemia: a report from the St. Jude Lifetime Cohort Study

Summary: The study examined the cumulative burden of chronic health conditions and neurocognitive and physical function among survivors of childhood AML treated with HCT or CT. It was found that both HCT and CT survivors had higher prevalence of certain health conditions compared to community controls, emphasizing the need for ongoing investigations of novel therapies to reduce late morbidities.

Noninvasive tools based on immune biomarkers for the diagnosis of central nervous system graft-vs-host disease: Two case reports and a review of the literature

Hai-Rong Lyu et al.

Summary: CSF and blood immune biomarker examinations played a crucial role in facilitating the diagnosis of CNS-GVHD, especially in critically ill patients requiring urgent treatment or those unsuitable for invasive diagnostic procedures. Both patients with a history of acute or chronic GVHD developed neurological symptoms that could not be explained by other causes, leading to a final diagnosis of CNS-GVHD based on a combination of clinical manifestations, CSF abnormalities, and negative findings of other potential causes.

Managing therapy-associated neurotoxicity in children with ALL

Deepa Bhojwani et al.

Summary: Various chemotherapeutic agents and novel immunotherapies are effective in treating leukemia, but can also be highly neurotoxic; the management of neurotoxicity requires vigilant monitoring, though symptoms are often temporary; it is crucial to resume critical medications after full neurologic recovery.

Summary: Neurological autoimmunity is a recognized complication of immune checkpoint inhibitor cancer immunotherapy, often presenting within the first three months of treatment. Diagnosis requires ruling out other causes, and treatment involves discontinuation or withdrawal of the inhibitor, possibly coupled with immunosuppression.

Severe allo-immune antibody-associated peripheral and central nervous system diseases after allogeneic hematopoietic stem cell transplantation

Martin W. Huemmert et al.

Summary: Allogeneic hematopoietic stem cell transplantation (alloHSCT) is an effective treatment for hematologic malignancies, but neurologic complications post alloHSCT are controversial. This study reports three cases of severe immune-mediated diseases of the peripheral or central nervous system following alloHSCT, emphasizing the importance of early diagnosis and treatment for improved outcomes.

Patient-Reported Outcomes for Cancer Patients with Hematological Malignancies Undergoing Chimeric Antigen Receptor T Cell Therapy: A Systematic Review

Mona Kamal et al.

Summary: Studies on the utility of patient-reported outcomes (PROs) in patients receiving CAR T cell therapy for hematological malignancies showed a high compliance rate, with an inverse relationship between fatigue and social function among adults. Cognitive difficulties and depression were common issues in adults, while emotional dysfunction improved over time in pediatric and adolescent patients. Quality of life status improved over time, but severe cytokine release syndrome and neurotoxicity caused delayed improvement. Further research with larger sample sizes and validated PRO tools at different time points is needed.
