The role of fibrosis index FIB-4 in predicting liver fibrosis stage and clinical prognosis: A diagnostic or screening tool?

Summary: This review evaluates the ability of the fibrosis index based on four factors (FIB-4) in identifying fibrosis stages, prognosis in chronic liver disease, and outcomes in acute liver injury. FIB-4 showed high accuracy in predicting advanced fibrosis in viral hepatitis and non-alcoholic fatty liver disease (NAFLD), but lower accuracy in alcoholic liver disease (ALD) and autoimmune hepatitis. It also demonstrated predictive value for hepatocellular carcinoma incidence and mortality, high-risk varices in cirrhosis patients, and long-term survival in hepatocellular carcinoma patients. In acute liver injury caused by COVID-19, FIB-4 had predictive value for mechanical ventilation and 30-day mortality. Finally, FIB-4 may serve as a screening tool for secondary prevention of NAFLD in high-risk populations.
